**HIP SPECIALIST**


**Description/Job Summary**


The overall purpose of the Hip Specialist is to improve the health of Perdue associates. The HIP Specialist will administer HIP at the facility to include administration of initial Health Risk Assessment (HRA), vital signs, measurements, lab draws, health
education & counseling, program assessment & evaluation.


**Responsibilities/Duties**


+ Perform interview for HRA and assist with completion of form. Measure vital signs, weight and height and record; Draw blood for appropriate/standard lab work (if competent), complete lab forms, prepare samples and send to LabOne.

+ Manage the implementation of all aspects of the health promotion program. Review results of HRA with associates, identify areas of risk and establish a Personal Plan For Health (PPFH) for the individual. Utilizing the principles of QIP, perform periodicfollow up of PPFH with associate.

+ Use and analyze data to assess and target health promotion needs for the employee population. Participate in marketing programs to increase associate awareness.

+ Provide support to Wellness Center/Occupational Health Department as directed by nursing supervisor to insure overall function of department.

**Required Qualifications**


+ Must be at least 18 years of age.

+ Legally authorized to work in the United States.

+ Strong work history, worked for one employer for at least 1 year in the past three years.

+ Graduate of accredited program of Professional Nursing, of practical nursing, or educational & employment related health promotion, (e.g. nutrition, exercise physiology).

+ If an RN or LPN, must have a current license in state of practice; occupational health and/or health education.

+ Bilingual (English/Spanish) a plus.

+ Clinical skills to include data collection, venipuncture, vital sign monitoring.

+ Excellent verbal & written communication skills.

+ Belief in improved health through reduction of controllable risk factors.
