VSC Transportation Coordinator
Assist residents in an Assisted Living Community with making physician appointments and provide transportation to Day Program or Workshop settings, as well as doctor and/or dentist appointments, etc.




+ Follow all regulations as required by the North Carolina Department of Health and Human Services, North Carolina Department of Mental Health/Developmental Disabilities/Substance Abuse Services and any other State or Federal Agencies as applicable


+ Understand and follow all regulations as required by Federal HIPPA Laws


+ Assist residents with making appointments and provide transportation to scheduled appointments, outings, etc.


+ Work closely with ED/Administrator/SIC/RCC. Notify him/her when there is a need for medical attention regarding a consumer or when there is a change in the consumer's health status. Relay information back to her from physicians.


+ Will be responsible for rotating On-Call (May include some nights, weekends, and holidays)


+ Be familiar with and adhere to all pharmaceutical policies and procedures.


+ Assist office manager when work is caught up.


+ File documents as needed.


+ Assure cleanliness of vehicles.


+ Assure vehicle inspections are renewed annually. Insure regular scheduled oil change of all vehicles.


+ Keep emergency fact sheets in all vehicles up to date.


+ Help in assuring that charts, ER files and all medical notebooks are kept up to date.


+ Provide transportation for pickups from hospital stays and to the hospital.


+ Chart in care notes when finished transporting to appointments.


+ Relay information to contact person after physician visits.


+ File in medical charts.


+ Help maintain the self-respect, personal dignity and physical safety of each consumer.


+ Work cooperatively with all other staff and relate favorably to consumers and families.


+ Report any accident or incident to the ED/Administrator or SIC.


+ Be familiar with and able to apply all the home's accident, fire safety and emergency procedures.


+ Operate office equipment as necessary (i.e., calculator, copier, computer, fax, etc.)


+ Assure careful treatment of equipment and utilization of supplies.


+ Be responsible for reading and adhering to all company policies and procedures.


+ Report to ED/Administrator/SIC/RCC any suspected abuse, neglect or exploitation of residents.


+ Attend job-related continued education and required training classes as needed. Understand the failure to renew classes by their due date will result in being removed from schedule.


+ Perform other duties as required by circumstances.


+ Be familiar with the Adult Care Home Residents' Bill of Rights and assist residents with exercising these rights.


+ Assure careful treatment of Adult Care Home equipment and utilization of supplies.








+ Must have a valid NC Driver's License


+ Must be able to read, write, understand and follow directions


+ Must have good written and verbal communication skills


+ Must be C.N.A or PCA certified


+ Must become Med Tech certified within 90 days of hire date


+ Must be able to Lift, Push, & Pull a minimum of 50 Pounds


+ Must have a two-step tuberculin skin test (first step prior to hire and second test within two weeks of first test)


+ Must have no substantiated findings on the NC Personnel Registry or Health Care Program


+ Must have a clear SBI and DMV record and maintain them during period of employment

+ Must have a valid NC Driver's License.

+ Must have had at least 8 years of driving experience, as this is a requirement of company's liability insurance carrier.
